Our solution was to install graphite-infused foam board on the poured concrete foundation walls.  This reduced the amount of sound migrating from the basement to the rest of the house.  As a bonus, it made this basement considerably more comfortable.

The attic was very poorly insulated.  We air sealed the attic then did an open blow of 17 inches of cellulose insulation.  The ice damming is no longer a problem.
